  •  Some qualifying conditions may require supporting medical documentation. Our team will

let you know if records are needed after reviewing your information. 


 

  • Go to Home page and select the green button that stats "Start Your Evaluation". 
  • This will take you to the contact page where you will select " Start the Process Today"
  • Fill in the following Information: Name, DOB, Email, Phone #
  • Our office will then review your information and forward you our intake paperwork to your email/phone #.


 

  •  The Intake Paperwork can take up to 20-30 minutes. 


 Patients must provide:

  • Valid New Mexico ID or Driver’s License 

Out-of-state or expired IDs are not accepted by the Department of Health and may delay the process.


 Patients must be 18 years or older. 


 Yes. All appointments are completed through telehealth. 


 Most insurance plans are accepted, including New Mexico Medicaid. 


Yes! Our process is done strictly online/Tele-health
